We Respect Your Privacy

We are committed to respecting your privacy and will only disclose personal information to ensure that you receive the best possible medical care.  When you visit our office, you will be presented with a complete copy of our Privacy Policy (which may also be accessed online here), and will be asked to sign a consent form for the use and disclosure of your information.

Central Missouri Physicians for Women is a HIPAA-compliant practice.

With your consent, we may use and disclose protected health information about you to carry out treatment, payment, and healthcare operations.

You have the right to review our Notice of Privacy Practices for a more complete description of such uses and disclosures.

With your consent, we may:

    • Call your home or other designated location and leave a message on voice mail or in person in reference to any items that assist the practice in carrying out your healthcare, such as appointment reminders, insurance items, and any call pertaining to your clinical care including laboratory results among others.
    • Mail to your home or other designated locations any items that assist the practice in carrying out your healthcare, such as appointment reminder cards and patient statements, as long as they are marked Personal and Confidential.
    • E-mail or fax to your home or other designated locations any items that assist the practice in carrying out your healthcare, such as appointment reminders or patient statements.

You have the right to request that we restrict how we use or disclose your protected health information to carry out your healthcare.
